Can sildenafil and dapoxetine be taken together? Not in Australia — not as one combined tablet, and not as two. No product combining the two medicines is on the Australian Register of Therapeutic Goods, and the Australian label for Priligy (dapoxetine) says it "should not be used in men with erectile dysfunction (ED) who are using PDE5 inhibitors", the class sildenafil belongs to (Priligy product and consumer medicine information). A combined tablet also locks both doses together.

Is a sildenafil and dapoxetine combination tablet approved in Australia?

No. The only dapoxetine product on the register is Priligy 30 mg, listed since 2 September 2010 (ARTG search: dapoxetine), and a search for both medicines together finds "no matching results" (ARTG search: sildenafil dapoxetine). Products that are not on the register "are not approved for supply in Australia, have not been reviewed for safety or quality" (TGA). Combined tablets do exist elsewhere: a study in Turkey used one containing dapoxetine 30 mg and sildenafil 50 mg (Tuken et al., Int J Impot Res 2019).

Why does the Priligy label rule the pair out?

Because together they may make you faint or dizzy on standing. Priligy "should not be used in patients using PDE5 inhibitors due to possible reduced orthostatic tolerance", the combination "may result in orthostatic hypotension" — a fall in blood pressure when you stand up — and its safety and efficacy "have not been established" (Priligy product information). Why can't the dose of a combined tablet be adjusted?

Because both doses are fixed in one tablet, and each medicine's dose is set by different rules. Sildenafil usually starts at 50 mg and can be raised to 100 mg or lowered to 25 mg, depending on how well it works and its side effects (EMA — Viagra overview). Its label suggests starting at 25 mg for men on alpha-blockers, men taking medicines that slow its breakdown (CYP3A4 inhibitors such as erythromycin) and men with severe kidney impairment (EMA — Viagra product information). Dapoxetine in Australia is one 30 mg tablet; the doctor checks your pulse and blood pressure lying down and standing before the first dose and reviews it after four weeks or six doses (Priligy product information), and it is ruled out altogether with strong CYP3A4 inhibitors such as ketoconazole (Australian Prescriber). A fixed tablet cannot follow both sets of rules:

  • it cannot drop to 25 mg of sildenafil without changing the dapoxetine too, and cutting it cannot separate the two medicines;
  • the timing differs — Viagra about one hour before sex (Viagra CMI), Priligy 1 to 3 hours before (Priligy product information);
  • if one medicine causes a problem, such as fainting on dapoxetine, it cannot be stopped while keeping the other.

What did studies of the combination find?

Short-term gains in small studies, with more side effects. In the Turkish study, 74 men with both problems took the combined tablet twice a week for four weeks, with no placebo group; 53 finished. Average time to ejaculation rose from about 23 to 68 seconds, and 10 of the 53 had side effects — most often headache, palpitations and flushing (Tuken et al.). A review of five randomised trials in 498 men without erectile dysfunction found dapoxetine plus a PDE5 inhibitor lengthened time to ejaculation more than dapoxetine alone, but headache was three times as common and flushing about 16 times (Mahesvara et al., Arch Ital Urol Androl 2025). None of this has changed the Australian label.

What has the TGA found in "combination" products?

Hidden sildenafil and dapoxetine. In June 2026 the TGA reported that testing of sexual enhancement products "that claimed to be dietary supplements or natural formulas" found "undeclared prescription-only substances sildenafil, tadalafil and dapoxetine"; one product held sildenafil and dapoxetine together, and none of the eleven was on the register. Such products "may contain undisclosed or harmful ingredients"; the TGA's advice is to stop using them and take them to a pharmacy for disposal (TGA safety advisory). What is used when both problems come together?

Usually the erection is treated first. Erectile dysfunction and premature ejaculation "frequently co-exist", as men with erectile dysfunction might try to ejaculate early, before losing the erection, and on their own PDE5 inhibitors do not lengthen the time to ejaculation (RACGP).
