What are the best male enhancement pills over the counter? None has been shown to treat erectile dysfunction, and in Australia every ED medicine needs a prescription (healthdirect). The pills sold without one are not ED medicines, and the Therapeutic Goods Administration (TGA) has repeatedly found prescription medicines hidden inside them: sildenafil, tadalafil and dapoxetine, undeclared on the label.
Is any over-the-counter enhancement pill proven to work?
No. Erectile dysfunction (ED) is treated in Australia with four medicines, sildenafil, tadalafil, vardenafil and avanafil, and each needs a prescription (healthdirect — Erectile dysfunction medicines). Pills sold over the counter for "male enhancement" are supplements, and none of the natural ingredients they list has been shown to work like those medicines: L-arginine and red ginseng have the most trial evidence, with small effects, and horny goat weed has been tested only in animals. What has the TGA found inside enhancement pills?
Prescription medicines, again and again. On 29 June 2026 the TGA reported the laboratory testing of eleven sexual enhancement products "that claimed to be dietary supplements or natural formulas": eight contained undeclared tadalafil and five undeclared sildenafil (two of them both), and one of the sildenafil products also contained dapoxetine. They were capsules, tablets, lozenges and a cream, and none was on the Australian Register of Therapeutic Goods (TGA safety advisory, 29 June 2026). On 8 July 2026 it added five sexual enhancement products labelled as "honey", all found to contain undeclared tadalafil (TGA safety advisory, 8 July 2026). All three medicines "can only be supplied to Australians with a prescription" (TGA).
The record goes back years. Searched on 27 September 2026, the TGA's safety alerts return 206 results that mention sildenafil, dated from June 2010 to June 2026, and 61 that mention tadalafil (TGA safety alerts: sildenafil; tadalafil). Most of the sildenafil alerts describe a product found to contain it undeclared, and the products are named in the alerts themselves.
Is it the same outside Australia?
Yes. A study of the US Food and Drug Administration's warnings from 2007 to 2016 counted 776 dietary supplements with unapproved drug ingredients, 353 of them marketed for sexual enhancement. In those, the most common hidden ingredient was sildenafil, found in 166 of the 353, and of 28 products named in repeat warnings more than six months apart, 19 were reported to contain new unapproved ingredients the second or third time (Tucker et al., JAMA Network Open 2018). The FDA says of its own list of contaminated sexual enhancement and energy products: "This list covers only a small fraction of the contaminated products on the market" (FDA).
Why is a hidden dose a danger?
Because you cannot follow a warning you were never given. Tadalafil taken alone or in combination with other medicines "can cause serious side effects", and it "may interact with nitrates found in some prescription medicines and may lower blood pressure to dangerous levels" (TGA safety advisory, 8 July 2026). No over-the-counter enhancement pill has been shown to treat erectile dysfunction, and TGA testing keeps finding, in some of them, the prescription medicines they are sold as an alternative to.
